This is got to be a joke...
First, you need to read the "What should your sheet look like" topic and put it on your arrangement.
Second, this is not a good piano format, this looks more like a duet than a solo piano.
Thirdly, what the hell is wrong with the 4th stave?! Put it in bass line please...

I won't go on for much longer, let the mods deal with this catastrophe...

Did you use a program to convert from a MIDI or something?

I don't see why anyone would abuse ledger lines in excess of ten lines for a single note... not to mention the fact that four staves are unnecessary for this song... and many other reasons that make me think a computer generated this...
