I finally played this game recently.

-The game name should be 'Metal Gear Rising: Revengeance', i.e. with the colon.
-The copyright should have more than just Konami in it.  Platinum developed it along with Kojima Productions if I remember correctly
-I don't hear these repeated Bs in the pickup.  I can hear F#s (and that would sound a lot less muddy).
-Your pickup is included in the bar numbering.  It shouldn't be.  I'll keep using your bar numberings though in this feedback to avoid confusion.
-The rhythm in bars 2/4/10/12 etc. sounds like in bar 6 to me i.e. a half note rather than two quarters.
-Missing a D in bar 8 in the right hand (resolves to the C# in the following bar).
-The left hand in bar 15 beat 3-4 sounds like G -> F# to me.  In bar 7, it could be B -> F# or G -> F# (not sure) but the note on beat 4 is definitely an F#.
-The only movement I hear in bar 17 is an E -> G - A on beats 2-4, not repeated Bs.
-I don't hear the D in bar 18.
-I'm not really sure what the glissando is meant to represent in bar 21.  I feel like something else would work better, like a tremolo or something.  There isn't really anything glissando-like in the original there.
-The melody sounds like the bass in bars 28-29, with the currently written in line being a harmony for it.  I would write it out like you did for the last two beats (GBG->C#AC#-> DBD etc.)  Or you could forego the octaves and just do thirds, I don't mind.
-I'm not sure what the RH meant to be doing on beat 4 of bar 36, it sounds like it's just sustained there until beat 3 of bar 37.
-I hear Fn as the bass note in bar 44, not the Cn.
-The RH in bars 44-45 sounds like full chords rather than power chords to me.

Ok I'm going to stop with the detailed chronological feedback here because we're not even off the second page and it's getting late here for me.  I'll give some more general feedback instead.  (Also I'm trying to refrain from giant posts....)

-Starting in 46 we have some issues with voicings that continue throughout the rest of the arrangement.  It's very unclear in a lot of places where the melody is and sometimes parts get blended together in a kind of confusing matter.  Nearly always the easiest way to have the melody stand out on piano is to have it be at the top of the chords that you are playing, or at least be obvious to the performer so that they can attempt to bring it out in the performance (or at least x2 so that they can follow it and understand what is going on in the piece).
-Secondly, 46 is quite underwhelming in this arrangement currently despite being the main theme of the whole track.  We go from these big chords with octaves and accents at fortissimo in the upper mid register to these low fourths and VERY low mostly single note left hand pattern.  It sounds muddy and not very bright at all.  I think putting the melody on top (see above) and up an octave could help this to stand out more.  Maybe mess around with the voicings a bit and see what you come up with.
-This track definitely loops in-game.  I think it should go back to bar 22 at some point around where the key change occurs in bar 134.
-Speaking of key changes, don't use a new key for only four bars (re bar 130).  It can just got to Bm there.
-I'm really not sure about those notes/voicings in bars 70-73.  It sounds waaay harsher there than the original and doesn't really gel with the rest of the arrangement either (those low notes are really too much in my opinion).  I'd specifically suggest having another look here.

Sorry I still haven't had time to address all the feedback. I'm trying to push this back toward the top of my to-do list, but I don't have as much time as I used to :(
