I lowered the sixteenth note accompaniment down an octave along with the melody when the melody changes instruments during the second pass. Sounds better than it overlapping the melody, in my opinion.

By your cross-staffing, is that just for ease of reading, or is does it signify which hand plays what? I ask because I've played this song (well, the OoT version) hundreds of times on the piano, so I've definitely experimented with all sorts of techniques.

Both! After experimenting a bit on my keyboard I figured after beat 2 it's not necessary to change the hand positions at all (just LH play what's on the bottom staff, and RH what's on top). This way also puts the gap between the Eb and F between the staves so you don't have to flip the notehead to the other side of the stem.
