Couple of things:
In bar 6, the G# should be an F#,
and the last note of bar 7 should be a C-natural, not a C#.

This may help.
Crank it up to 720p/1080p and you can probably just transcribe the notes and arrange as necessary.

.... but, i do have a jazz band arrangement of it. at least the first half, before the tempo changes and gets slower.

may I have that?
I'm in my college's Jazz Band and i'd like us to play it. I don't think the second half of it would work in a jazz band anyhow.
I can always re-arrange/tweak your arrangement if needed.

Actually that one is completely wrong.
It doesn't even appear to be from FFIV at all.
