Measure 1: The Dynamic here should be in the center of the measure.

Measures 2-4, 6-8 &, 10-12: F Sharps in these measures should be G Flats.

Measures 13, 14 &, 18: I'd put slurs on these notes. One slur in each measure covering the whole measure would be nice.

-Title on page 2 is missing (top of the page.)

-I'm not too sure about the bass in this song. You might wanna take another listen.

-Measure numbers are missing.

The entire bassline was missing past m. 13+. There's no way to know if this was an intentional omission from LifeMushroom (a decision I would respect) seeing as he hasn't been around for several months, so I will assume it was simply an error on his part and edit the OP with the final version below:
