I looked, and I could NOT find this music anywhere, so I had to base it off memory. With that, nothing sounded inaccurate. Here's what I fixed:
-I copied everything over into the template file for NSM
-I fixed the copyright
-I widened the distance between the two staves by .1" to give the dynamics a bit more room to breathe.

I'm not able to search right now, but perhaps the sheet is mistitled?

Unrelated, but making the systems 3-4 measures instead of 4-3 may distribute space a little better.

That may be it, but I can't find an OST list. And alright, I'll experiment with that when I get home. Do I need to bring the distance between the staves back down to 1"?

Ok, I have updated things a bit. Firstly, I redistributed the measures and reset the distance between the staves. Secondly, after asking around, nothing has been concluded, so I'm assuming there's no official OST name for this theme (and several others). Given this, I changed the name to "Delfino Police" (thanks, TZP!) (Can this be changed officially, please?) Also, TZP (thanks again!) gave me an isolated MP3 file for the theme, given as the only things YouTube and elsewhere produced were this theme with, say, the Episode preview theme.

Some thoughts:
Your sheet looks great!
You seem to have added a few extra notes. Granted, they are hard to hear. Also, the low note (beat 2.5 and 4.5) are actually the same note as 1.0 etc.
I think it'd be better as an octave lower.
To give you a good picture of what I'm talking about, this may help:

Basically, I took out the notes that aren't in the original. I also changed the right hand to the bass clef and added "Edited by E. Gadd Industries."
