I need the following sheet layout...

hiya, ok, i just recently got a challenge from one of my friends who has the music for "To Zanarkand". He told me the layout for the piece and game me a midi of it and told me to get to work to see how close i can get to it, the thing is, i cant do the format on Notepad so i need someone to make this sheet for me, please.

It is 46 measures long, starts with both clefs being Treble, it is treble cleff from measures 1-10, then Treble-Bass from 11-29, Then Treble-Treble from 30-37, then from 38-46 it goes to Treble-Bass. The Key is in G (the sharp being F if for some reason you dont know) and if its possible (i dont know) i need the following right-hand measures at 8va from 1-4, 9-11, and 30-33. Thats what he told me. I would like to quickly get to work so please make this for me in all haste if you can. Thank you.

I was unable to put the 8va because Songwriter doesn't have it, only Printmusic and above does. Is that ok?
